WHY IS THE CESAREAN DELIVERY RATE INCREASING?

- Increased old cesarean section: The cesarean rate increases because patients who had previous cesarean were obliged to have a cesarean section in their later pregnancy.
- Expansion of cesarean indications: In the past years, some cases that were not justified for cesarean section have been considered as cesarean indications today. For example, the baby is breech (coming upside down).
- Today, due to the development of the conditions, techniques and technology related to surgery and anesthesia, cesarean is considered a more reliable surgery and is not avoided until the old years.
- Increased cesarean delivery depending on patient request
- With legal concerns, physicians prefer the chance of normal birth or prefer cesarean instead of applications such as vacuum and forceps.
- The cesarean is often more advantageous for the doctor and the hospital. Caesarean section may be the reason of preference in physicians' intensive working conditions, especially since it takes less time.
- Not giving the midwives sufficiently effective role in pregnancy follow-up and birth.
- The public is not provided with information and training on issues such as the advantages and disadvantages of normal delivery and cesarean delivery. People getting wrong information about these issues from their surroundings and unreliable sources.
- Marriage and birth ages progress: Today, women are getting married at a later age due to their education and work conditions. In older women, the need for cesarean increases due to blood pressure, diabetes and other health problems.
- Increased cesarean rates due to IVF applications. One reason for this is the idea that pregnancies occurring as a result of difficult efforts with in vitro fertilization are delivered without risk by cesarean, which is not true. Another reason is that twin and triplet pregnancies are higher in pregnancies with IVF.
- The prevalence of obesity (overweight) today is one of the reasons that cause a predisposition to cesarean section. It is difficult for obese women to give birth normally and extra problems such as blood pressure and diabetes due to obesity can also cause cesarean.
- The fact that the application of painless delivery (epidural anesthesia) is not widely disseminated is one of the reasons for expectant maternal pain fear cesarean increase.
- Fear of expectant mothers that their genital organs may be impaired if they give birth normally is a reason leading them to cesarean section.
- Prospective mothers thinking that they may have urinary and stool incontinence problems if they give birth normally: There are many studies on this subject, but there is no clear evidence that optional caesarean section prevents these diseases. Today, scientific evidence does not recommend cesarean delivery to prevent urinary and stool incontinence problems.
Research shows that continuous electronic fetal monitoring (continuous nst and baby heartbeat monitoring) increases the caesarean rate during normal delivery. However, in the same studies, it is shown that continuous monitor follow-up does not benefit in subjects such as perinatal infant death, cerebral palsy, neonatal intensive care and apgar score. Again, many studies have shown that there is no difference between performing continuous electronic fetal monitoring during normal delivery and performing intermittent auscultation (listening to the baby's heart beat).
